EllisDon Forming Ltd. and two supervisors were fined after two separate roof collapses occurred during the installation of composite floor systems at two construction projects, causing minor injuries to workers. The company failed, as an employer, to ensure that the measures and procedures prescribed by section 31(1)(b) of Ontario Regulation 213/91 were carried out, contrary to section 25(1)(c) of the Occupational Health and Safety Act. Additionally, two supervisors, contravened section 27(1)(a) of the Act.
